This interactive session will help attendees make the most of their PCMH transformation by harnessing the power of the NCQA data requirements. Attendees will receive tips and tools to make their PCMH data engaging to staff and patients. The session will be structured to be flexible enough to answer the highest demand questions of the audience, but systematic enough to have attendees at any level walk away with a game plan.
